By booking a consultation at Ascend Chiropractic, you acknowledge the following:

• Cancellation: We request at least 24 hours’ notice for any cancellation to allow other patients to be seen.
• Arrival: Please arrive on time. Late arrivals may result in a shortened consultation time to respect subsequent appointments.
• Payment: Fees for services are due at the time of the appointment unless otherwise arranged in a package plan.

3. No Doctor-Patient Relationship

Using this website or communicating through it does not establish a formal doctor-patient relationship. Professional advice is only provided after a face-to-face clinical assessment.
